Have you ever had a patient who had a healthcare proxy assigned? In the case of a patient being unable to consent to medical procedures, a healthcare proxy is an individual designated to make medical decisions on a patient’s behalf. In order for a healthcare surrogate form to be active, it has to be signed by two witnesses, one of whom must not be related by blood. In the event the healthcare surrogate is unable or unwilling to act as the decision-maker, the hospital can seek another proxy on the patient’s behalf.
